Research published in the latest issue of the British Dental Journal suggests that people with severe dental phobia may be able to overcome their anxieties with a single session of cognitive behavioural therapy (CBT). Cognitive-behavioral therapy is based on the concept that changing negative thinking patterns and self-defeating behaviors can have a powerful effect on the patient's emotions.

CBT is a structured therapy, which involves a partnership between the client and therapist. Together with the therapist, clients examine all elements that maintain a problem, including their thoughts, feelings, and behaviors.
A survey on adult dental health published earlier this year by the NHS Information Centre suggests that as much as 12 per cent of people may experience extreme dental anxiety. 60 patients who relied on intravenous sedation before they were able to undergo dental treatment were surveyed. The study concluded that the benefits were of such significance that the authorst of the study advise dental providers to implement this approach now rather than wait for the results of further research.
“Dental phobia is a serious problem because it deters some people from ever going to the dentist, except when they are in severe pain,” Prof Damien Walmsley, scientific advisor to the British Dental Association, said. “At this stage, they may require more invasive treatment than might be the case if they went to the dentist regularly. Sadly, this cycle of anxiety, non-attendance and pain is often repeated in the children of those with dental phobia, perpetuating the problem and feeding another generation of oral health problems.”
The benefits of CBT for severe dental phobia appear to endure over time, the authors of the study, who work at the Sheffield Health & Social Care NHS Foundation Trust and Heart of Birmingham Teaching
The initialgroup 60 patients had all attended a specialist dental clinic in Sheffield for people with severe dental phobia, Similar to State of the Art Comfort Dentistry in Smithtown, NY. Half the group were offered CBT, of which 21 accepted the treatment. Twenty of these went on to have dental treatment without having to be sedated. An audit of these patients a decade later found that of the 19 patients who had had CBT and could be located, none had returned to sedation in the intervening ten-year period.
