  To answer this question we need to go back…back in time.   You may not realize this, but tooth decay was virtually nonexistent 10,000 years ago.  In fact, tooth decay is a relatively modern disease when we look at the 2 million years humans have been living and evolving on this planet.  Tooth decay as well as many other diseases are what used to be called “diseases of civilization” until it became politically incorrect.  However, it is a very fitting name and it includes cardio-vascular disease, strokes, heart attacks, many cancer, obesity, high blood pressure, diabetes, food allergies, and autoimmune diseases such as Lupus, Alzhiemer’s, multiple sclerosis, vetiligo, psoriasis, etc.

     Approximately10,000 years ago humans began transitioning form living as hunter-gatherers to farming.  Agriculture has turned out to be a double edge sword. It allowed humans to develop civilization and technology at the expense of our health.  Today at the highest point of human civilization and technological achievement we are sicker than we have ever been before and it is going to get worse!  We went form a diet that we evolved to eat for 2 million years, a diet that gave us our large and impressive brain, to a diet that in the short 10,000 years of agriculture and processed foods that is full of foods that are foreign to our metabolism.  

 We evolved eating a diet of meat, fish, poultry (and the fat that came along with these animals) and seasonal veggies and fruit.  Skeletons of our ancestors showed perfectly straight teeth with zero cavities.  These skeletons also showed people with strong robust bones with muscle insertions similar to today’s top athletes. Of the little over 200 remaining pockets of hunter gatherers left today none of these people show any signs of tooth decay or any other disease of civilization. 

     Our agricultural diet is full of foods that we are not designed to eat. These foods include grains, legumes, dairy, plant oils and exorbitant levels of fructose found in processed food and unnaturally sweet fruit that are available to us year round.  The unnaturally enormous levels of carbohydrates as well as antinutrients and toxins found in these “evolutionarily foreign” foods are making us sicker and sicker. 

     Grains and legumes are the seeds of plants. Since plants have no claws, teeth or legs the only defense against their seeds being eaten is biologic chemical warfare: toxins and antinutrients. Enough of these chemicals survive heating and digestion to wreak havoc on our health.  Dairy is also an unnatural food.  We are the only mammals on this planet that drink the milk of other mammals into adult hood.  Milk makes babies grow and cow babies need to grow FAST! Not only is milk loaded with proteins that are toxic and allergic to humans, it is full of natural (and sometimes unnatural) growth hormones.   Plant oils are loaded with omega 6 fatty acids that are pro-inflammatory and unnatural to our diet. Fruits have been bred to be large and sweet and are now available all year and together with High Fructose Corn Syrup found in everything form ketchup to soda are overloading our liver with fructose and destroying our health and teeth.

About Dr. Saul M Pressner

I have been in practice since 1981 and am highly trained in all phases of dentistry. I graduated from the University of Pennsylvania School of Dental Medicine and have my certificate in Family Dentistry from the University of Connecticut School of Dental Medicine. I am a Fellow of the Academy of General Dentistry and take many continuing education courses. Anxiety Free Dentistry – Frequently Asked Questions on Reducing Dental Anxiety

 Q. Why do you need to have regular dental care check-ups?

A. Regular check-ups are needed to monitor your overall oral health. In addition to checking for cavities, your dentist examines the health of your entire mouth and surrounding soft tissues, checking for pre-cancerous or cancerous lesions, oral sores, and gum disease.

Your oral health is connected with your general health. Dental care check-ups can alert the dentist to other medical conditions that have symptoms in the mouth such as diabetes, nutritional deficiencies, and hormonal irregularities. Regular dental care visits are vital to prevent tooth decay, gum disease, and other conditions affecting your mouth.

Q. What can you do to feel more relaxed during a dental care appointment?

A. With the combination of modern anesthetics and new conscious sedation dentistry technology and techniques, many procedures only have minimal discomfort or are now entirely painless. Dental care providers want their patients to have maximum comfort and approach their treatments with a relaxed attitude and less dental anxiety.

There are a number of ways to decrease dental anxiety:

Tell the dental care staff your concerns. Studies have shown that talking about your fears of possible discomfort actually reduces pain.

Use visualization to picture yourself relaxed, or repeat messages that have a calming effect.

Play your favorite music on a walkman you bring with you (some dental offices are equipped with walkmans, too.)

Arrange a signal with the dental care staff, such as raising your hand, if you need a break or feel discomfort.

Some dental care offices are now offering patients new options for stress-relief: hypnosis, self-hypnosis instructions, relaxation tapes, soft lighting, warm gel-filled eye masks, scented candles, and massaging pillows. These are helpful in reducing stress in patients that suffer from dental anxiety. Be sure to avoid the use of stimulants such as caffeine prior to your visit.

Q. Aromatherapy has a relaxing effect for many people. Is this effective for patients undergoing dental treatment?

A. Research studies conducted at Case Western Reserve University have noted that the use of aromatherapy has a significant positive effect on dental anxiety patients. Two-thirds of the patients receiving aromatherapy were more calm and relaxed than those patients without exposure to the scented fragrance oils.

Dentists are concerned about your comfort. Ask your dental provider if aromatherapy is available in the office to help reduce dental anxiety, or if you can bring your own for your dental care visit. Essential fragrance oils are available in health food stores, spas, and some grocery and drug store outlets.

Q. Are dental patients who are considered at risk for bacterial infections advised to take antibiotic medication prior to their appointments?

A. Certain medical conditions, such as heart valve problems or a recent total joint replacement, are considered at risk for infection at the site of the cardiac abnormality or joint replacement. This infection results from bacteria from the mouth entering the bloodstream and working its way to these vulnerable areas.

Consequently, dental care procedures likely to result in bleeding from the gums or mucous membranes will require patients to take antibiotics prior to that procedure. Such procedures could include, but are not limited to, extractions, implant surgery, incision and drainage for oral infection, and professional teeth cleaning.

Guidelines have been established by the American Heart Association and the American Dental Association to provide dentists and physicians with information regarding appropriate regimens for antibiotic therapy. It also outlines those situations when antibiotic therapy is or is not indicated.

There also are other medical conditions warranting antibiotic therapy prior to dental procedures. Be sure to update your dentist regarding your medical history. Your dentist and/or physician will advise you of any special needs.

Q. Where can you receive dental services if you do not have the money to cover the related expense?

A. If you do not have either dental insurance or the money to pay for a dental visit, you should inquire about financial aid from various sources within your community.

You may need to make several calls, but the local dental society, the local public health department, or social service agencies may be able to direct you to sources of assistance. Also, check local hospitals, dental schools, and outreach clinics that may be able to provide dental services at a reduced fee.


I’m proud to offer you laser dentistry for the treatment of many types of cavities. The laser works by emitting a specialized light that targets and destroys dental decay.

Drills are very powerful, and the vibration and the large hole produced can cause discomfort, making some type of anesthesia almost always necessary. With a laser, the light works to destroy and remove decay with no uncomfortable tooth vibration and resulting discomfort. Because the laser is so precise, the decayed part of the tooth often can be selectively removed, leaving a more healthy part of the tooth. Rather than that loud drilling sound, you hear only a gentle tapping.

Dr. Shvartsman of Smithtown, Long Island focuses on treating people wiyh dental phobia.  The entire staff is comitted to patient comfort.

     Research published in the latest issue of the British Dental Journal suggests that people with severe dental phobia may be able to overcome their anxieties with a single session of cognitive behavioural therapy (CBT).   Cognitive-behavioral therapy is based on the concept that changing negative thinking patterns and self-defeating behaviors can have a powerful effect on the patient's emotions.  

  CBT is a structured therapy, which involves a partnership between the client and therapist. Together with the therapist, clients examine all elements that maintain a problem, including their thoughts, feelings, and behaviors. 
 

     A survey on adult dental health published earlier this year by the NHS Information Centre suggests that as much as 12 per cent of people may experience extreme dental anxiety.  60 patients who relied on intravenous sedation before they were able to undergo dental treatment were surveyed.  The study concluded that the benefits were of such significance that the authorst of the study advise dental providers to implement this approach now rather than wait for the results of further research. 

     “Dental phobia is a serious problem because it deters some people from ever going to the dentist, except when they are in severe pain,” Prof Damien Walmsley, scientific advisor to the British Dental Association, said. “At this stage, they may require more invasive treatment than might be the case if they went to the dentist regularly. Sadly, this cycle of anxiety, non-attendance and pain is often repeated in the children of those with dental phobia, perpetuating the problem and feeding another generation of oral health problems.”
 

     The benefits of CBT for severe dental phobia appear to endure over time, the authors of the study, who work at the Sheffield Health & Social Care NHS Foundation Trust and Heart of Birmingham Teaching 
 
     The initialgroup 60 patients had all attended a specialist dental clinic in Sheffield for people with severe dental phobia, Similar to State of the Art Comfort Dentistry in Smithtown, NY. Half the group were offered CBT, of which 21 accepted the treatment. Twenty of these went on to have dental treatment without having to be sedated. An audit of these patients a decade later found that of the 19 patients who had had CBT and could be located, none had returned to sedation in the intervening ten-year period.

Dr. Alex Shvartsman is a top graduate of SUNY at Stony Brook School of Dental Medicine. He also holds a BA degree in Biology. His natural-born artistic talents and passion for dentistry allow him to excel as a dentist. 

After completing his residency at North Shore University Hospital at Manhasset, Dr. Shvartsman was appointed Chief Dental Resident. In July of 2010  Dr. Alex Shvartsman, has received the prestigious Mastership Award in the Academy of General Dentistry by completing an additional 600 hours in each required dental discipline. 400 hours were in hands-on or participation courses.  He is one of a little over 1% of American dentists who have achieved this honor. Mastership in the AGD is a symbol of a dentist’s continual journey toward improvement, innovation and progress. He currently serves as a peer reviewer of articles for the AGD monthly dental journal. Dr. Shvartsman’s commitment to staying current in the rapidly changing field of dentistry is exemplary.

The Syrijet is a device that adminisgered dental local anesthetic with a puff of air instead of the dreaded needle.  Patients that are afraid of needle injections in their mouth really love this device.  For everyone the injection process is painless.  Patietns experience a rapid pop form the puff of aneshtetic that is delivered via air pressure into the gum. 


Digital 3-D X-Rays

In his ongoing effort to bring you the best of what dentistry has to offer,  Dr. Shvartsman is the first general dentist on Long Island to offer the benefit of Digital 3-D x-ray scans to his patients.  The Galileos Cone Beam system was installed May, 2010.

 Now Dr. Shvartsman’s patients will fully benefit  from the revolutionary cone beam Digital 3-D technology of the Galileos. At State of the Art Comfort Dentistry the doctors and dental hygienists are no longer limited to interpreting 2-D x-rays and make “educated guesses”.  A quick 14 second outside the mouth scan is all it takes. Patients will not have to suffer through 18-22 uncomfortable individual inside-the-mouth x-rays. This is especially great news for people with a high gag reflex, small mouths or who have difficulty taking conventional x-rays.

With the Galileos images Dr. Shvartsman can see inside the bone and teeth in 3 dimensions, making diagnosis more exact and accurate.   This translates more predictable and safer treatment outcomes. The Galileos gives vastly more information with 4 times less radiation than a complete set of digital x-rays and 80-100 times less radiation than traditional CAT scans. In addition, both jaws and the sinuses are clearly seen with the 3-D Galileos scan, while a complete set of 18-22 individual 2-D x-rays only show the teeth and a little bit of the surrounding bone.  In fact, physicians such as ENTs, are now referring their patients to Dr. Shvartsman for Galileos scans.

Laser Dentistry

Dr. Shvartsman of Smithtown, Long Island, uses Dental Lasers as part of his dedication  to providing you with the most comfortable dental care possible. That is why we offer Laser Dentistry as an alternative to traditional techniques. We currently use 4 different lasers in our laser dental practice.

Laser Cavity Scanner: The Diagnodent
Most dentists use a sharp-pointy pick to check for cavities. You may have this instrument stuck in your tooth in the past. Not only can this be painful, but a sharp-pointy pick coming at your face is not a comforting experience if you are already nervous at the dentist. The fact is cavity detection with this technique is only about 60% effective; many small cavities go unseen and can become large cavities if left untreated. This can lead to unnecessary root canal treatment, crowns or extractions.

In our office we use the Laser Cavity Scanner called the Diagnodent. This laser is over 90% effective in painlessly finding event the smallest cavities early, when they can be treated conservatively with a small filling. In many cases you do not even need a shot! We are thrilled to offer this gentler and more effective laser dentistry diagnostic tool to our patients.

Laser Fillings: The Er:Yag Dental Laser

This dental laser allows us to treat cavities without the use of shots and Novocain. Over 90% of patients do not feel anything at all during treatment, and those who do say it wasn’t bad enough to need a shot. You can have a filling done and go have a meal, give a speech or meet with a client without the worry of looking and sounding funny from a numb lip and face.

Gum disease is caused by bacteria deep under your gums and around your teeth.  Moderate and advanced gum disease is traditionally  treated with surgery whrre the diseased gum and bone is surgically cut away. Although this procedure shrinks the pockets the tooth roots are exposed. Many patients complain of tooth sensitivity nad increased root cavities anas well as esthetic concerns.

This technology is extremely effective at vaporizing diseased gum tissue and bacteria from around your teeth. A Biologic adhesive clot is then developed with the laser to help reattach the gums back to your teeth.
